ABSTRACT

Diabetes Mellitus (DM), like other non-communicable diseases accounts for a great number of deaths in Cameroon, Africa and the world at large. A person dies from Diabetes every 7 seconds. Diabetes Mellitus accounted for an estimated 4.9 million deaths worldwide in 2014, and the causes of death were due to DM itself or diseases associated to DM. Several factors were related to deaths in diabetic patients.
To determine the mortality rate, causes of death and to assess the factors related to death in type 2 diabetic patients receiving care in Douala General Hospital (DGH).

We carried out a retrospective cohort study at the DGH. The mortality rate, causes of death and factors related to death were assessed using Hospital records of patients who consulted from January 2009 to December 2014 and had been followed up for at least 12 months. WHO Verbal Autopsy instrument through phone calls was implemented to get patient’s cause of death, if patient was dead. The data was analysed using SPSS version 20, Kaplan-Meier survival curves plotted and Cox regression analyses was used to determine factors related to death.
Amongst the 628 patients recruited (352 Males), 54 had died giving a mortality rate of 8.6%, within a median follow up period of 3.1(1.9-4.5). 55.6% of the deaths were males. Acute metabolic coma, Cardiovascular Disease (including strokes) Nephropathy, Cancer, Diabetic Foot Syndromes and others (trauma), accounted for 22.2%, 16.7%, 14.8%, 14.8%, 14.8%,13.0% respectively. Using multivariate analyses, Age (>57years) and Glycated Haemoglobin (>6.5years) were all significantly related to death.

Mortality rate in type 2 Diabetic patients is high. Diabetic patients die both from diabetes related and non-diabetes related causes. Age and HbA1c are the major determinants of death.
