ABSTRACT

Despite the increase accessibility of antiretroviral treatment, increase of deaths from HIV related Liver diseases is becoming a big challenge to the management of HIV/HBV co infected patients in hepatitis B endemic settings. To prevent liver diseases from taking an edge over success of antiretroviral therapy in Cameroon, there is a need for evidence based policy amendments in National HIV patient care guidelines and policy implementation concerning, the type of drugs administered, The administration sequence of drugs and the provision of free HBV screening and treatment in HIV patients. In this study we shall enroll 1500 HIV infected patients,Men ,women and children from seven out of the ten regions of Cameroon with highest HIV prevalence;South 7.2%, East 6.3%, North west 6.3% Center 6.1% southwest 5.7%, Adamawa 5.1% and littoral with 3.9% with 4.6% in Douala .After obtaining ethical clearance, administrative authorizations and participants’ informed consent, samples will be collected and screened for hepatitis B surface antigen HBsAg then positive samples further screened for HBeAg anti HBc, anti HBe, anti HBs antibodies and subsequently HBV DNA sequencing. In this study we intend to to determine the prevalence of HBV/HIV co-infection by age, sex and Region of origin,to characterize co-infection through screening of hepatitis B serologic and molecular markers,to determine the effect of co-infection and antiretroviral therapy on the liver by measuring and comparing mean serum transaminases, alkaline phosphatase, total protein and albumin levels in patient with HIV mono and co-infection and to determine the effect of co-infection by comparing CD4+T cell counts in co-infected and mono infected as well to identify risk factors associated with liver disease in both mono and co-infected.This research seeks to provide baseline information on the national prevalence and sero-epidemiology of HBV/HIV co-infection and the effect of HIV, co-infection and antiretroviral therapy on the liver of HIV patients. We expect that study results will bring about implementation of policy which will limit complications due Liver Disease in HIV patients
